Community Nurses are qualified nurses who have specialist understanding of caring for patients at home and in the community.
We aim to work with you and your family/carers to plan and agree an individual program of care.
Where appropriate the Community Nursing team will help you to become as independent as possible.
This will involve teaching you and your carers how to manage your condition.
Student, (nursing and medical) may accompany the Community Nurse on occasion with your permission.
Home visits are provided according to genuine need. Difficulty with transport to and from your surgery is not a valid reason to receive a home visit.

Our midwife provides antenatal care, looks after expectant mothers who have a normal pregnancy and cares for each mother and baby in the postnatal period.
If you wish to make an appointment with Julie, then please telephone the practice 0115 855 42 50 – Sr Elderton clinics take place on a Friday morning and all day on Thursdays.
Our midwife is employed by Derby Hospitals Foundation Trust.
Ask Sister Elderton for details of the antenatal classes when you see her for your first appointment.
